Release
I hereby irrevocably grant to American Cancer
Society Inc., its legal representatives or assigns and those acting under its permission and upon its authority, or those for whom American
Cancer Society Inc. is acting, the absolute right and permission to record my and/or my child’s likeness on film or in testimonial (written
words), to edit or change or alter such recording(s) at its sole discretion and to copyright and/or use such images (s) in which I may be
included in whole or in part or composite or distorted in character or form, or reproductions thereof in color or otherwise for art,
advertising, trade or any other lawful purpose whatsoever in any media throughout the world, in perpetuity.
It is also my understanding that I will receive no
monetary compensation for my likeness or testimonial. I hereby waive any right that I may have to inspect and/or approve the finished product
or the advertising copy that may be used in connection therewith or the use to which it may be applied.
I hereby release, discharge and agree to save
harmless American Cancer Society Inc., and its employees or agents, legal representatives or assigns and all persons acting under its
permission or upon its authority or for whom its is acting, from any liability by virtue of any blurring distortion, alteration optical

The Fund Raiser
What is the “Cover Kid” Contest?
The contest is a fund raiser for the American Cancer Society Relay
For Life in Ouachita Parish. Cover Kid Contest is an online “voting” contest for the opportunity to raise money for the American Cancer
Society. The entrant raising the most money will be featured on the cover of Louisiana Road Trip magazine’s May 2010 issue. The children
placing in the overall top 10 will have their photos printed inside the same issue.
Who does the “Cover Kid” contest benefit?
All entry fees and online donations benefit the American Cancer
Society Relay For Life of Ouachita Parish. Each child may designate an area Relay For Life team to support on his/her entry
form.
Entering the Contest
Who can enter?
Children age 12 or under by May 1, 2010 may enter the contest. The
child(ren) entering the contest must be residents of Louisiana. A photo entry may feature up
to 2 siblings. If 2 children are featured in one photo – they must be siblings. If a photo
featuring two siblings is the winner, they will be photographed together. Pets are not eligible.
Is there an entry fee?
The entry fee is $10 per child. If a photo entry features one child – the entry fee is $10. If a photo entry features 2 siblings – the
entry fee is $20. Entry fees are not included in the contestant’s “vote” count. The entry fee does not count toward the donation total in
the contest.
How do I submit an entry?
Mail or deliver entry form with entry fee to the American Cancer
Society at 1761 North 19th Street, Monroe, LA 71201. Photos will be accepted via
e-mail or attached to the entry form. E-mail photo to brooke.henry@cancer.org . In the email, please include child’s name. Entries
will not be posted if the fee is not received in the ACS office by February 15, 2010. No entries
accepted after February 15, 2010 at 5:00 pm CST.
Are there requirements for the photos?
Photos should be no larger than 5x7 and no smaller than wallet
size. (Note: All photos will be electronically cropped by the American Cancer Society to meet size and space requirements of the online
site.) If photos are emailed, they must be sent as attachments, not included in the body of an email. Photos will not be
returned.
Portraits taken by a professional photographer, studio or business
such as Wal-Mart or Sears, must be accompanied by written permission or a copyright release. Otherwise, the American Cancer Society will be
unable to post the photo. Candid digital photos are encouraged.
Is there an entry deadline?
Photos will be accepted for entry from January 4-February 15,
2010. No entries will be accepted after February 15 at
5:00 pm CST. Photos will not be returned.
Why is an e-mail address required?
This contest is conducted online and email is integral to its
success. You can increase votes for your child with links and emails that can be shared with your family and friends. To actively
participate, you will need a computer, email and internet access. Email will be the main
method of communication between entrants and contest officials.
Cover Kid Voting
Voting Page Set-up
Once an entry is received and the entry
fee paid, an online donation page will be set up for each entrant. Entries with two siblings will be included on one donation page.
American Cancer Society staff will set up each page and contact the entrants via e-mail with the donation page web page address and other
details. Online voting with donations begins on February 1 and ends on March 15 at 5:00 pm CST. Only the entrant’s first name will be
included on the online page.
How are votes placed?
Beginning February 1, you can visit the Relay For Life Cover Kid
Contest website at http://www.relayforlife.org/ouachitala and vote for your favorite
kid.
Donations may also be made by printing off the donation form from
the entrant’s online voting page and delivering or mailing it with a check to the American Cancer Society office in Monroe.
How will the winner and runners-up be
determined?
The contestant that raises the most money for the American Cancer Society will be
designated as the winner and will be featured on the cover of the May 2010 issue of Louisiana Road Trips magazine. Nine runners-up will have their photos printed inside the magazine in a story featuring Relay For Life
and the Cover Kid Contest.
The most money raised will be determined by calculating the total donations processed and
recorded through the online voting site by 5:00 pm CST on March 15, 2010. Please note that it may take the financial institutions several minutes or even
hours to process the approval on credit and/or debit card transactions, so a donation placed on the Web site just before the
cut-off, may not necessarily be approved by the financial institution and recorded by 5:00 pm. Only donations documented by our Convio system as processed and
approved by 5:00 pm CST on March 15 will be counted. Note: An email receipt generated by the system is not the official record of the
transaction time stamp.
Donations made by submitting a check and an offline donation form
must be received at the American Cancer Society office in person, by mail or other delivery method no later than 12 noon CST on March 15,
2010 in order to allow staff time to record the donation on line before close of business that day. These offline donations will not be
seen in the contestant’s total online until they are posted by the American Cancer Society office.
Any donations that qualify for an employer matching gift must be
accompanied by the proper forms when delivered to the American Cancer Society. An official matching gift form does qualify as a gift and
will be included in the totals.
Because these Relay For Life events continue to raise money online
past the contest and until their Relay date, the voting pages CANNOT be disabled or shut down immediately after the contest. There will not be an automatic end to the ability to donate on the entrant’s page at 5:00 pm CST on March
15, 2010. Please tell people voting for your contestant that any donations that receive financial institution approval after the
cut-off time will not be counted in the Cover Kid totals. However, all donations received are still tax deductible gifts to the American
Cancer Society regardless of the approval time.
Final winners will be notified by telephone on or around March 17, 2010.
Photography of the
Winner
Photos of the winner(s) will be taken by Patty Stewart
Photography. The winner will be contacted to set a date and time for the photo sitting.
Louisiana Road Trips magazine and the American Cancer Society will
determine the appropriate attire and background for photo that will appear in the magazine.
Final selection of the photos for the magazine is determined by the American Cancer Society
and Louisiana Road Trips magazine.
